2010 CAMP WORKERS
Thank you for interest in working at Indian Lake Nazarene Camp this summer!

Here are instructions for the 2010 Summer Volunteer/Worker Application plus Health History Form.

1.  Please complete the worker applicaton and health history form;
2.  Sign both documents;
3.  If the applicant is a minor under age 18, please download and complete a minor work permit form;
4.  If required, be sure parent or guardian signs worker application, health history, and minor work permit;
5.  Complete the applicant information on 3 copies of the Reference Form (page 6);
     a.)   Give one copy of the reference form to your pastor to complete,
            and a copy to two non relatives to complete; and THEN
     b.)  Have the completed reference forms mailed directly
           to appropriate camp director.
6.  Mail all other applicant completed documents to appropriate camp director.

Page 5 of the application provides name and mailing address for each Camp Director or Coordinator.
